<p><strong><i>Question by Tony</i>: Should I go back to college now?</strong><br />
Lots of stuff has happened in my wee little life.  Parents were drug heads, grew up with my maternal grandparents both passed away by the time I was 16, then lived with paternal grandmother till 18.  My grandmother(mom&#8217;s mother) was diagnosed with a brain tumor and she was my best friend and always wanted me to strive for the best in all things and I did my best to help her through radiation treatments and brain surgeries and she is now gone.   My mother all throughout my childhood was in and out of jail, my father too.  They finally would get stable and tug on me to come and live with them which would mean I&#8217;d leave my grandparents behind.  My mother is in prison now, and my father is a house &#8220;dad,&#8221; while he is with a wonderful, very accomplished woman he baby-sits and helps at the house.  Anyhoo, I always participated in sports, and strived for exceptional grades in fact, I graduated second in my class.  I had 15 hours of college credit and I went big and shipped off to UT Austin.   I guess I was supposed to be the God-child of the family.  Church events, sport events, community events, top of my class, going to UT gonna become an anesthesiologist and everything great!  Well&#8230;I made it to UT&#8230;started off fine and was shaken.  I had all A&#8217;s for the first month&#8230;B&#8217;s second month&#8230;and 3rd F&#8217;S!  I went to office hours, studied with friends, went to tutors.  500 pages to be read(write two essays over and take a test by Friday) a week, massive homework in statistics every night with a mini-test every friday&#8230;constantly making presentations and preparing for that massive 20 page research paper in another class&#8230;and worrying about the ONLY 4 tests i had the in the other class.  I was overwhelmed, scared, and alone?  I would study for 100 hours(no exaggeration)  at least 10 hrs a day for 10 days for one test that i made a 55 on.  That happened on three occasions&#8230;so it was like a horrible nightmare i couldn&#8217;t comprehend the material no matter how hard I tried, was depressed, had anxiety attacks, lost 24 pounds, and went to the mental health center on campus but felt like I got the whole generic &#8220;how do you &#8220;FEEL&#8221; about that psychological treatment.&#8221;  Nehoo, dropped out about a month before finals right after finding out i had failed 2 of my classes.  (not to mention was fighting with my family because to THEM I WAS GOING TO GRADUATE WITH HONORS!).<br />
-anyways, after all that..I really want to get back into school.  I took the next semester off, went back in the fall of last year barely scraped a C out of one class and a D out of the other(easy classes).  I guess I lost my motivation.  I lazily didn&#8217;t get signed up for classes this spring&#8230;and am now working a part-time job.  I had a really hard time with those classes last fall&#8230;I would get super anxious about easy tests and assignments and my UT college experience would come back and haunt me, get dreams about it and all.   I mean is there still hope?  After 4 F&#8217;s in classes at my previous university(all a&#8217;s from the community college I attended back home), and the C and D at this community college.   Is there a way I can still attend finish it out and go to nursing school like I had intended all along?  I feel like there&#8217;s no hope.  And this time I want to go on MY wants and dreams not my parents and families.  Sorry it&#8217;s so long&#8230;guess my original question sums it up should I still go back and even if I do will i make it far?</p>

<p>Problems continue for GlaxoSmithKline. The company delivered a poor set of results for 2004. While this was said to be largely due to competition from generic medicines in the pharmaceutical sector, the company&#8217;s &#8220;promising&#8221; new product pipeline is not delivering products quick enough to fill this gap.</p>

<p>Acquisitions</p>

<p>GlaxoSmithKline has regained the sales and marketing rights to five US prescription-only dermatology products from Elan Pharmaceuticals. The five brands are: Aclovate, Cutivate, Emgel, Oxistat and Temovate and have sales of around $  35 million. Elan had licensed the brands from Glaxo Wellcome but decided against extending the agreement as it is withdrawing from dermatology. GlaxoSmithKline says that the brands, although prescription, make an excellent fit with the OTC brands in the Oxy portfolio and with the Abreva cold sore cream..</p>

<p>Warning in Canada</p>

<p>GlaxoSmithKline has warned wholesalers and retailers in Canada that they have to cease supplying prescription drugs to US patients as they are substantially cheaper in Canada. GSK says that its action is because of safety concerns for consumers who could be receiving drugs that are damaged during shipment or the prescription of which is not properly supervised by a doctor. The principal target of GSK&#8217;s action is Web-based pharmacies. Annual sales of prescription drugs to the US from Canada&#8217;s Manitoba area are estimated at some Â£300 million. Ten leading US and Canadian healthcare and business organisations started a national advertising campaign in the New York Times that encouraged consumers to look to reverse the ban. The advertisement also encouraged them to sell any GSK stock that they owned and to switch from GSK OTC products to comparable brands from other companies.</p>

<p>Challenges in generics</p>
<p>&#13;</p>
<p>The FDA has received applications from companies that are looking to start manufacture of generic versions of the migraine drug Imitrex and herpes drug Valtrex. The patent for Valtrex runs until 2009 while Imitrex lasts until 2008.</p>
<p>&#13;</p>
<p>GlaxoSmithKline still has some breathing space before these drugs are vulnerable to generics.</p>

<p>The first quarter of 2005 was relatively quiet in the pricing, reimbursement, and prescribing environments of the worlds major pharmaceutical markets. The United States continued the implementation of the Medicare Modernization Act; the acts new prescription drug benefit is expected to give a substantial boost to pharmaceutical sales. At the same time, some members of Congress, state governments, and large employers pursued measures to reduce prescription drug prices in the United States. Several leading pharmaceutical companies responded to growing criticism of their prices by launching a new drug discount card. In Europe, governments sought ways to refine their well-established cost-containment strategies. Reference pricing provoked controversy in France and Germany, and the relatively underdeveloped generics markets of Southern Europe continued to grow. In Japan, industry observers saw signs of increasing pressure for reform of the pricing and reimbursement system. </p>
<p>&#13;</p>
<p>Business Implications</p>
<p>&#13;</p>
<p>   The new Medicare prescription drug benefit is scheduled to begin operation on January 1, 2006. Prescription drug plans (PDPs) will be required to cover at least two drugs in each therapeutic category and pharmacological class. However, if a given category or class contains only two drugs, the PDP may limit coverage to a single agent within that category or class. Plans will not be obliged to cover off-label prescribing, and they will be permitted to use mechanisms such as prior authorization, step therapy protocols, generics substitution, and quantity limits to restrict access to certain drugs. PDPs will have to promote the use of generics by requiring the display of information on less expensive generic versions of a drug (if available) at the point of sale. &#13;<br />
&#13;</p>
<p>   The pressure to legalize drug importation in the United States may become irresistible in the coming months. Two drug importation bills have been revived in Congress. A sponsor of one of the bills predicted that legislation to legalize drug importation will be passed by a margin of 75-25 votes, a majority that would make it difficult for the Bush administration to maintain its opposition. The pharmaceutical industry’s best hope of avoiding massive drug importation in the United States may be that Canada will act to ban this trade and that other potential parallel export markets will follow suit to protect their pharmaceutical supply systems.&#13;<br />
&#13;</p>
<p>   The European Commission recently reaffirmed its resolve to implement the recommendations of the G10 Medicines Group. If the commission keeps its word, manufacturers can look forward to faster registration and market access for innovative drugs, streamlined pricing and reimbursement decisions at the member-state level, free pricing for drugs that are not purchased or reimbursed by member states, easier Rx-to-OTC switching and the freedom to continue using the same trademark for drugs switched to nonprescription status, the abolition of restrictions on the advertising of nonprescription medicines, and the introduction of measures to limit parallel imports from new member states in Central and Eastern Europe. &#13;<br />
&#13;</p>
<p>   The impact of Germany’s expanded and more aggressive reference pricing system will be studied with interest not just in that country but throughout Europe. Many governments have followed the lead that Germany set in 1989 by introducing reference pricing, but none have been as bold as the pioneer of this approach to cost containment. Pfizer and other companies that have taken legal action against reference pricing decisions will anxiously wait to see the outcome of their lawsuits in Germany. The pharmaceutical industry as a whole will anxiously wait to see if other European governments decide to extend reference pricing to patent-protected drugs.&#13;</p>

The U.S. markets sluggish performance is attributable in part to the very cautious attitude of the FDA toward the reclassification of prescription drugs as nonprescription medicines, a process known as &#8220;Rx-to-OTC switching.&#8221; However, recent statements made by the FDA indicate that its attitude toward Rx-to-OTC switching is about to change.</p>

<p>We begin this report by considering the perspectives on self-medication of three major stakeholder groups: consumers, payers, and manufacturers, plus the position of the FDA. We then examine the cost-containment implications of Rx-to-OTC switching in two drug classes that have had recent switch approvals (nonsedating antihistamines and proton pump inhibitors) and two classes that may see switches in the near future (statins and obesity therapies). We conclude with a brief assessment of the outlook and implications for the pharmaceutical industry.</p>

<p>Although the default status of medicines in the United States is nonprescription, the Food and Drug Administration (FDA) has traditionally been very cautious in regard to Rx-to- OTC switching (i.e., the reclassification of prescription drugs as nonprescription medicines). Far fewer compounds are available in OTC form in the United States than in the other major self-medication markets. However, the FDA has stated that it intends “to become more proactive in recommending key potential switches.&#8221;</p>
<p>&#13;</p>
<p>Given that prescription drug prices are generally significantly higher than the prices of comparable OTC medicines, manufacturers usually have a vested interest in maintaining the prescription status of their new chemical entities as long as possible. However, as a drug approaches patent expiration and the threat of generics competition, it may be advantageous for the manufacturer to switch the drug to OTC status as a way of extending its life cycle. The timing of the switch is crucial: it should be early enough to develop consumer loyalty to the OTC brand but late enough to exploit the full sales potential of prescription sales.</p>
<p>&#13;</p>
<p>When a drug is switched to OTC status, health plans often end reimbursement of other drugs within the same class or move these drugs to a higher formulary tier that incurs copayments higher than the OTC drug’s retail price. On average, copayments for third-tier drugs (i.e., nonpreferred brands) exceed $  30 per month. The substantially lower prices of OTC drugs compared with competing prescription drugs have persuaded many health plans to add certain switched drugs to their formularies, often in a new tier that is subject to a very modest copayment (e.g., $  5). According to a recent managed care survey, 31.5% of HMOs covered at least some OTC medicines in 2003.</p>

<p>Recent FDA judgments suggest that the pharmaceutical industry may need to be somewhat bolder in its dosing proposals in switch applications. In the past, manufacturers have assumed that reducing the dosing from standard prescription levels would enhance their prospects of succeeding with their switch applications, but the opposite has been the case in some instances. The FDA has shown that it is unwilling to approve switches that may use subtherapeutic dosing. </p>

<p>Sawai Pharmaceutical was established in Osaka, in 1929. The company was incorporated in 1948, and set up manufacturing lines for OTCs in 1961. In 1965, the company changed from manufacturing OTCs to ethical pharmaceuticals. Sawai was listed on the OTC stock market in 1995, and was first listed on the Tokyo Stock Exchange in 2000.</p>

<p>The company deals in the export and import, manufacturing and sales of pharmaceutical products and healthcare foods. Its main products are cardiovascular drugs, gastrointestinal drugs and other metabolic drugs. Its key customers are hospitals, clinics and dispensing pharmacies across Japan.</p>

<p>In January 2006, Sawai announced it had won a lawsuit filed by Eisai regarding Sawai’s teprenone product, Seftac. The lawsuit had been brought before the Tokyo District Court in March 2005, and was based on Japan’s Unfair Competition Prevention Law.</p>

<p>In July 2006, Sawai’s shares surged on market rumours that the company was shortly to be the subject of a takeover by Teva. Sawai quickly denied the rumours, although Teva had yet to comment at the time of writing. </p>
